VACCINATIONS
- Must bring an updated shot record to the appointment if the child has received vaccines elsewhere.
- A parent must be present to sign the consent for vaccines for children under 18 years old. If another family member must bring the child to their appointment, they must have a note from the parent or legal guardian giving that person consent to sign for vaccinations. They also should list on the note any allergies to foods or medications for the child, any previous reactions/problems with vaccines, or any history of taking a steroid in the past month for the child. We must be able to reach a parent by phone to obtain verbal consent as well.
- For more information on vaccination schedules or questions related to vaccine safety, see www.cispimmunize.org/IZSchedule_Childhood.pdf
- For Vaccine Information Statements, see http://www.cdc.gov/vaccines/pubs/vis/default.htm. Vaccine Information Statements (VISs) are information sheets produced by the Centers for Disease Control and Prevention (CDC) that explain to vaccine recipients, their parents, or their legal representatives both the benefits and risks of a vaccine.
