VACCINATIONS

• Must bring an updated shot record to the appointment if the child has received vaccines elsewhere.
• A parent must be present to sign the consent for vaccines for children under 18 years old.  If another family member must bring the child to their appointment, they must have a note from the parent or legal guardian giving that person consent to sign for vaccinations.  They also should list on the note any allergies to foods or medications for the child, any previous reactions/problems with vaccines, or any history of taking a steroid in the past month for the child.  We must be able to reach a parent by phone to obtain verbal consent as well.

• For Vaccine Information Statements, see http://www.cdc.gov/vaccines/pubs/vis/default.htm.  Vaccine Information Statements (VISs) are information sheets produced by the Centers for Disease Control and Prevention (CDC) that explain to vaccine recipients, their parents, or their legal representatives both the benefits and risks of a vaccine.

 LEAD TESTING

• Recommended for children at 9 months old and 24 months old
• Also recommended for any child under 6 years old that has never had lead testing or has risk factors for lead exposure.

 FLUORIDE VARNISHING

• Recommended every 6 months for all children from the time they get their first tooth until 6 years old

 SCHOOL AND SPORTS PHYSICALS

• Sports physicals and school physicals for a child with insurance must be done by our Family Nurse Practioner.
• All other school physicals or well-baby check-ups on those without insurance may be done by one of our RN’s specially trained for Well-Child Exams. Medicaid children are eligible for this service as well. 

EARLY AND PERIODIC SCREENING, DIAGNOSIS AND TREATMENT (EPDST) 
Well Child Visits

Early and Periodic Screening, Diagnosis and Treatment (EPDST) is a program for children from birth to age 21 who are eligible for Medicaid or birth to age 19 for the Kentucky Children’s Health Insurance Program (KCHIP). Under the EPDST program, children are checked for medical problems early. There are specific tests and treatments as they grow older. If a problem is found, you will be referred to your doctor for treatment and further evaluation of the problem. These check-ups are provided at no cost to Medicaid and KCHIP-eligible children. The areas of health care that are checked include:

• Medical history and physical exam
• Growth and development check
• Vision and Hearing
• Teeth
• Nutrition
• Lab tests including blood lead level
• Mental health and substance abuse
• Immunizations
• Health Education for parents

Children should get health check-ups regularly or before the ages listed below:
