Fort Campbell WIC Food Program – For ALL Military who live in Kentucky on OR off the Fort Campbell base.

A federally funded supplemental food program for infants and children, pregnant, postpartum, and lactating women. Participants must be income eligible and at nutritional risk. Participants receive nutritional education along with vouchers for basic nutritional foods to supplement their diet.
Before you or your children can be screened for WIC we need to see proof of:
Your Identity – bring one of these for yourself and the person being screened
- Current Medicaid Card
- Birth Certificate
- Driver’s License
- Social Security Card
- Work or School ID
- Voter Registration Card
- Hospital Birth Record
- Immunization Record
Your Residence – bring one of these
- Current Medicaid Card
- Current bill for electric, gas, telephone, or cable
- Current lease or receipt
- Driver’s License
Your Household Income – bring all sources of income for all household members
- Current Medicaid Card
- Food Stamp Letter
- Current Pay Stub
- Signed Statement From Employer
W-2 Forms
- Income Tax Forms For Most Recent Year
- Unemployment Letter/Notice
- Check Stub/Award Letter From Social Security
- Recent Leave and Earnings Statement (LES) for Military
- Foster Child Placement/Award Letter
- Tax Forms or Accounting Records for Self-Employed
- Copies of Alimony or Child Support Checks
- Statement From Person(s) That Provide You Money/Resources
** Remember**
If you or anyone that lives with your receives Medicaid, bring the Medicaid card to the clinic. This card may meet the proof requirements.
Proof of household income must be provided when you have the purple KCHIP III card.
If you do not have proof and cannot get proof, please let us know.
